could it be possible to change the tempo of an mp3 song in realtime, using the mad ? (i mean tempo = like in the MIDI format - not changing the pitch but just the speed that the musical notes being playing.)
can this change within the mad code cause heavy latency ?

Sounds like somthing outside the scope of the decoder. Once you have the PCM you can do whatever you want to it, including pitch shifting (which is what I assume you are referinfg to).
