This usually means MAD found some garbage in the stream, but it could also be caused by the presence of an ID3 tag. I tried to improve this in 0.10.0b so madplay doesn't flag ID3 tags as an error, but I might not have it perfect.
I also have a suspicion that certain ID3v1 tag writers *may* corrupt the last audio frame in the file, or at least not put the tag immediately after it. YMMV, and I'd be happy to hear what anyone knows about this.
