Is it possible to make MAD decode at an arbitrary sampling frequency? ie for slowing down/speeding up the audio stream. I know I could resample after decode, but if MAD could do it as part of the decoding process that would be much better (and also would update timers, etc.)?

While it is possible to tell MAD to decode at a sampling frequency exactly half of the original, it's not a trivial matter to decode at any arbitrary sampling frequency; this is not supported.
It's probably best to explicitly resample after decoding.
The HQ DSP option was a contemplated option that never materialized.
