There is sample code distributed with the MAD tarball. See minimad.c for a really stripped down example. Madplay.c (and its associated files) show a more "full-bodied" example of the high-level interface.

Additionally, there is an example of the low-level API on the web here: http://www.bsd-dk.dk/%7Eelrond/audio/madlld/
